Pediatric Board Certification
- We take care of newborns through age 18 years
- We accept most insurances but are not currently accepting Medicaid
- Accepting: Blue Select, Blue Cross-Blue Shield, Cigna, UHC, Aetna
- We typically can accommodate same day sick appointments
*Note: We are not accepting patients who do not vaccinate, or who follow an alternative schedule that will not complete the CDC recommended schedule by 24 months of age.
*Note: Please cancel or reschedule appointments at least 24 hours in advance or you may be charged $50.
If you are going to be 10 minutes or more late for your appointment, please call the office at (850) 678-9009. Your appointment may need to be rescheduled.
*Note: Please complete, print and bring forms needed for your visit.
Services include, but are not limited to:
- well child/sports/school/daycare entry physical exams (Form)
- sick and injury visits
- all CDC/AAP recommended vaccines
(including current flu immunizations)
- prenatal visits *no charge
Dr. Burton-Lindner is on-call
when the office is closed.
